Innovatief project van zorggroep Huisartsenzorg Drenthe
De Academische Werkplaats Duurzame Zorg werkt mee aan een innovatief project van zorggroep Huisartsenzorg Drenthe (HZD). Het doel van dit project is door middel van een ziekte-overstijgend, integraal programma chronische zorg, de zorg te verbeteren voor patiënten met (meerdere) chronische aandoeningen. Om meer inzicht te krijgen in de doelgroep van een dergelijk programma, is in het afgelopen jaar de zorgzwaarte van de patiëntpopulatie in Drenthe onderzocht.
De resultaten van deze fase van het onderzoek zijn gepubliceerd in het rapport “Op weg naar een integraal programma chronische zorg: Resultaten deelonderzoek in de zorggroep Huisartsenzorg Drenthe.” Het vervolgonderzoek, gekoppeld aan een promotietraject, zal zich de komende periode richten op risicostratificatie en waardecreatie voor huisartsenpraktijken.
