"Evolution of complex endovascular aortic repair"
The thesis summarizes the critical evolution of complex endovascular aortic repair during the last two decades. The thesis is elaborated in four parts which represents transition periods by the authors experience. In Part I, the technique of physician modified endovascular grafts (PMEGs) and the transition to company manufactured fenestrated and branched stent-grafts is discussed, outcomes. In Part II, the contemporary results of the authors experience and multicenter experiences in the United States Aortic Research Consortium are discussed with emphasis on improvements in mortality, risk of spinal cord injury and comparisons with open surgical repair. In Part III the main cause of secondary interventions, which is target vessel complications are discussed and in Part IV, the effect of sarcopenia and changes in quality of life measures. Overall, the thesis compiles the most important contributions by the author in the field of fenestrated-branched endovascular repair (FB-EVAR) which is now accepted as an alternative to open surgical repair in patients with suitable anatomy.
