"ABCC: Easy as 1-2-3? Studying the implementation of a digital innovation in Dutch general practices"

Implementing change is effortful but worth the effort. This thesis describes the development, validation and implementation of the Assessment of Burden of Chronic Conditions (ABCC-)tool. The ABCC-tool measures and visualizes the burden people experience because of their chronic condition. As such, the ABCC-tool aims to facilitate a personalised approach to chronic care. Bringing innovations to daily practice is challenging but essential for the tool to benefit its users. These challenges lay in understanding the context, user experiences and implementation readiness. In other words, understanding how, where and by whom the tool is going to be used. This thesis highlighted that implementation plans are crucial for effective implementation. These plans should at least include skill-based training, and implementation guidance, and should always be directed at local needs. Both the ABCC-tool and the implementation studies in this thesis stress a personalized approach through assistance and guidance of those in need of help.
