Neurovascular diseases are disorders of the large and small blood vessels in the brain. These can involve cerebral infarcts and brain haemorrhages. Although the consequences of disorders in the smallest brain blood vessels are no less than those in the large blood vessels, much less attention has been paid to the disorder of the smallest brain vessels. Therefore, I dedicate my inaugural address to the microvessels of the brain.

In my talk, I will first take you briefly through the acute treatment of cerebral infarction of the large blood vessels, and the major developments that have taken place here in recent years. Then we move on to the smallest cerebral blood vessels. Apart from cerebral infarction and cerebral haemorrhage, microvessel disease can also lead to cognitive decline and dementia. I will show how our scientific research is helping us understand better and better what happens in this condition. This is necessary to start taking the next step: intervening in the early disease mechanism.
