Requesting computer glasses
If an employee meets the conditions as laid down in the UM regulation reimbursement computer glasses, the employee could be eligible for a reimbursement of the computer glasses. The employee makes an appointment with the prevention officer. The prevention officer will state that computer glasses are necessary and the employee will undergo an eye examination by an optician or optometrist to determine what computer glasses are needed. The optician or optometrist will provide the employee with an explanatory report and an original and specified invoice.
In most cases, computer glasses are regular glasses consisting of regular lenses in a simple frame. As a rule, the costs of the lenses do not exceed €300 and the price of the frame does not exceed €75 (both inclusive of VAT). The purchase takes place based on the report by the prevention officer and the recommendation by the optician/optometrist. For trendy frames costing in excess of €75, the additional cost will continue to be at the expense of the employee.
If special computer glasses are necessary on medical grounds, the company doctor should advise on this. The employee only has to make an appointment with the company doctor if the reimbursement of the computer glasses will exceed the maximum amount of € 375, -. An appointment can be made by sending an email to mijnafspraken[at]arbounie[dot]nl with subject computer glasses Maastricht University. The company doctor has to provide the UM employee with a written confirmation on medical grounds that these computer glasses are necessary.
The department where the employee is employed will take care of the further administration and reimbursement. Please note that the reimbursement in principle will only be granted if the employee will provide his/her department with the original and specified invoice and the explanatory report drawn up by an optician or optometrist.
